Centres intégrés universitaires de santé et de services sociaux (CIUSSS) West-Central Montreal is organizing a pop-up vaccination clinic on campus. 

The clinic is only for Concordians who need their first or second dose of the COVID-19 vaccine.

Bring your RAMQ card OR a piece of photo identification, such as your passport if you are not from Quebec. 

Please note you'll need to wait a minimum of four weeks between your first and second dose.

For students vaccinated outside Quebec and wanting the Quebec Vaccine Passport (VaxiCode):  Please note the Concordia clinic is not equipped to register vaccines from outside Quebec for VaxiCode app (vaccine passport) purposes.

We encourage you to go to a public vaccination clinic like the Décarie Square Vaccination Clinic (6900 Décarie Boulevard) to register your vaccine with the Quebec Vaccination Registry.

You don't need an appointment, but you can book one at clicsante.ca. Choose COVID-19 vaccine troubleshooting.

Bring these documents with you:

  • Hard-copy proof of your vaccine(s) administered outside Quebec
  • Government-issued identification (e.g. passport or study permit)

You'll need to know your Montreal address (proof not required).

It may take a few hours to get your QR code.
