Cancellations
If you wish to cancel your appointment, please call us at 514-848-2424 ext. 3565 as soon as possible. You can call 24/7 to cancel. Leave us a detailed voicemail message with your name, ID number, date and time of appointment and who your appointment is with.
To cancel an appointment with a psychiatrist, call by 10 a.m. at least two working days in advance.
For all other care providers, call by 10 a.m. at least one working day in advance.
Confidentiality
Legal information you have to know
The information contained in the medical records of Concordia University Health Services is confidential. Medical information will not be shared without the consent of the patient, except if required by law (e.g., risk of suicide, child endangerment, a subpoena).
Note: personal information may be shared among Concordia University Health Services staff as required.
The patient will have to sign a Release of Information form before Concordia University Health Services communicates any medical and/or personal information or documents with people, units or institutions outside of Health Services.
A patient who requests a copy of, or information from, his/her own medical file should direct such requests to a nurse. Please note we charge a fee for these requests. Refer to medical records for more information.
Our commitment to confidentiality applies to all settings — clinical, research and administrative — in which Concordia University Health Services creates, receives, uses, processes, maintains or furnishes health information to patients.
Medical records
It’s your file! We take your confidentiality very seriously.
We are happy to provide you or your medical care provider with a copy of your medical records.
In most cases, if these records are for a medical care provider, we can send them for free. For personal copies of your records there is an administrative fee that you pay in advance by credit card:
A fixed fee of $20 for the first 20 pages and then a $0.50 fee for any additional pages
Additional $20 fee for archived files (files are archived after three years and kept for five years only)
How to get a copy of your medical records
Complete the release of information form, and email it to Health Services at healthservices.info@concordia.ca to request your file. You may ask for a complete copy or a specific part of your records (eg. from a specific timeframe).
A nurse may contact you to validate your identity and instruct you about the process. Payment must be made by credit card.
Certain delays may apply depending on the size and location of your file. For mental health records, we may have to check with the psychiatrist before releasing the file.
