How to make a request
Online
Using your student email account, log in to Office 365 and then follow the instructions for the type of request you wish to make, as indicated below.
In person

Type of request
- You can request an attestation letter online (Mobile | Laptop) or in person.
- The “instantaneous pdf fast-track status letter” is the most common letter. It serves to confirm student enrolment and is available for printing immediately after the request is placed. Please click “view report” on the page after your instantaneous fast-track letter request is placed to access your letter.
- In certain cases, enrolment letters require additional information and must be issued by our team. These are called “conventional letter full-time/part-time letters” and are issued within 5 business days.
- We issue these letters without the university stamp, unless you provide acceptable written justification for a stamp. Your letter, which will appear on university letterhead, will show the signature of the Registrar or a designate of the Registrar.
- Alumni can request a letter confirming their credential by emailing students@concordia.ca.
- A $20 (CAD) fee applies.
- You can request a letter online via Office 365.
- We issue these letters without the university stamp, unless you provide acceptable written justification for a stamp. Your letter, which will appear on university letterhead, will show the signature of the Registrar or a designate of the Registrar.
- In the case of a degree completion letter, it will indicate you completed your degree requirements, but have yet to be awarded your degree.
- A $20 (CAD) fee applies.
- Allow at least ten business days for delivery.
- If you need a university representative to fill out an enrolment form, and/or add a university seal, email the form to the Birks teams at students@concordia.ca.
- A $20 (CAD) fee applies.
- We will return the form to you by email as a PDF attachment within five business days.
- You can request unofficial transcripts online via Office 365 or in person.
- A $5.95 (CAD) fee applies.
- If you submitted your request online, we will email you a PDF attachment within three business days. In person, we issue unofficial transcripts on-the-spot.
- Unofficial transcripts are only issued to the requestor. Our team does not send them to third-parties.
- If you have questions about your Quebec residency situation, we can discuss with you by phone or in person.
- If you need to make a sworn statement to a Commissioner of Oaths, you can do so in person at Birks.
- The Société de transport de Montréal (STM) uses your student ID card photo to create your transit pass, the OPUS card.
- You may submit an OPUS card request 24 hours after the university issues your student ID.
- To qualify for an OPUS card, you must have a home address in Quebec saved in your Student Centre and you must be enrolled in full-time studies. Note that wait-listed courses do not count towards enrolment.
- The STM will mail your OPUS card to your home address.
- Request a student OPUS card. You can also request a student OPUS card by logging in to the Student Hub and going to My CU Account.
- Follow the instructions and make a payment of $15 to the STM via the OPUS platform.
- Another option: print an OPUS attestation letter (per the attestation letter instructions, specifying OPUS letter) and then take it to the STM photo studio at Berri-UQAM metro station. Note that you may need an appointment.

- Only for undergraduate students.
- If your professor agreed to accept your course work after the term has ended, you may submit a request for a late completion grade. If your student record does not already reflect the incomplete (INC) notation, you'll need to submit proof of your professor's approval.
- You can request a late-completion grade online via Office 365.
- The deadlines to request late completion are February 1 for a Fall course; May 15 for a Fall/Winter or Winter course; and September 1 for a Summer course.
- A $30 (CAD) fee applies.
- Graduate students wishing to request late completion may do so by submitting a Service Request for In-Progress Extension.
- In the event you feel your course work (tests, exams, essays, etc.) was unfairly graded, it is your right to request academic re-evaluation.
- Re-evaluation should be a last recourse, after attempting to resolve the matter informally and directly. When possible, please ask to review your work with your instructor prior to submitting this request.
- Should you choose to pursue academic re-evaluation, you can make the request online via Office 365. As part of your request, please describe any informal efforts that you made with your instructor so as to resolve the matter, as well as why you would like to proceed with a re-evaluation request.
- The deadlines for submitting an academic re-evaluation request are February 1 for a Fall course; June 15 for a Fall/Winter or Winter course; and October 1 for a Summer course.
- A $47.50 (CAD) fee applies.

- You can request official transcripts online. The university will mail your transcripts, or for an additional fee, send them by courier service. Students may also pick up their sealed official transcript at Enrolment Services FB‑900 (1250 Guy street).
- If you request that your official transcript be sent to another university in Quebec, Concordia will send the transcript electronically through the BCI network.
- A $14.40 (CAD) fee applies.

- You can find your free account summary online easily.
- Log into the Student Hub, then navigate to My CU Account > Student Centre > Student Services > Account inquiry.
- An account summary is not a document that includes a university stamp.
- To request a name change in the strictest confidence, please email name.change@concordia.ca.

Receiving your diploma
- If you are an upcoming graduate, the university will email you detailed instructions.
- If you have recently graduated, see the diplomas page for more info.
Replacement diploma
- You may request a replacement online or in person at Birks.
- A $100 (CAD) fee applies.
- As part of your request, you'll need to make a sworn statement, which you can do in person at Birks.
Certified copy
- You'll need your original diploma (or a replacement diploma) so we can make a certified true copy of it.
- You can get a certified copy by mail or on the spot at Birks.
- A $10 (CAD) fee per copy applies.
Here's how you can go about making these requests:
- Annual tax receipts for Educational Tax Credits (Federal T2202 and Provincial Relevé 8) are available on or before February 28 of each year for the course fees paid in the previous taxation year.
- Find your receipts via the Student Hub under My CU Account > My Student Centre > Finances, then click the horizontal lines icon, then choose Tax Receipts.
