Incident report form instructions
To assist with your report, please provide your full name, email address, and contact information so a CSPS agent can follow up with you. If you are unsure of any requested details, leave those fields blank. Providing inaccurate or misleading information may delay the processing of your report.
Within 48 hours of submitting an online incident report, a CSPS security agent will contact you to verify the details, ask any necessary questions, and provide a case number if required.
Anonymous reporting:
You may submit a report without providing personal information. However, if you choose to remain anonymous:
- CSPS will be unable to follow up if additional details are needed.
- The ability to take action may be limited if critical information is missing.
Important Note:
Online submissions are not official reports. This form helps security agents gather key details quickly. Based on the information provided, CSPS will prepare an official incident report with a case number, which will be logged in the reporting system.
